A player cannot receive a tournament guest player card if the player has already been registered to a team during the current seasonal year. 

Go to osysa.com click on forms and resources (top right corner) and select All OSYSA forms

Scroll down to 2nd link read instructions then click Tournament Guest Player Card Link.
