Player Transfer 

If the Player is transferring from a CUSL club, the player is NOT to be entered into the

CUSL computer System as an original player they are to be entered as a transfer.

If they are coming from another league then enter them as an original player, Add the transfer player

to the CUSL website using the “Add a player when registration is closed” selection. 

(The player will not show up on roster) E-mail the Club Administrator and request that your 

Player Card be printed. The Administrator will need the Team Name, Number and Player name 

to complete this process. You will be notified when the card is ready for pick up. 

For A CUSL to CUSL transfer player you will need: 

·    Old Player Card from previous team

·    New Player Card (with small picture glued on. head only) 

·    Transfer paperwork found at osysa.com under Forms & Resources Tab, select All OSYSA Forms,

    Scroll down to Other Forms:  Click on 

    Ohio South Inter Team Player Transfer Request and Approval

   Must be original (NOT a faxed copy, emailed copy or regular copy

• PLAYER MUST sign the form, not the parent

• Coach from previous team must sign the form
