For An Out of State player you will need: 

For Kentucky Player (processing for the new season starts June 1st)

·    Go to www.kysoccer.org website

·    Go to Administrative

·    Select Forms

·    Click on  Out of State Player

·    Create an Account or Log in If you already have one.

·    You can pay the $20.00 Fee by Credit Card

·    Information needed to complete the form:

                          League: Cincinnati United Soccer League

.    You print or save the form (use the "Save As" option)

.    You can email the form to cwsc.webmaster@fuse.net OR

	Fax to 513-251-2348 OR

	Include it in your Registration or Roster Change Packet.

For Indiana Player (processing for the new season starts July 1st)

This processes take a few weeks so please plan accordingly

·      go to www.indianayouthsoccer.org website

·      Under Services click forms

·      Under Registration Forms click

		Resident Player Registration – Permission to play on an Out of State Team  

·      Print and complete the form

·      mail to:     Indiana Youth Soccer
                 5440 Herbert Lord Rd.
                 Indianapolis, IN 46216
  or fax to 317-829-0555 (contacts 800-347-4972 or 317-829-0560)

·      include the form with the $10 fee (can be charged over the phone)

·      Information needed to complete the form:

               Club Name: Cincinnati West Soccer Club
               League: CUSL
               Age Group: ____________
               Return Completed Form To: Can be coaches address, CWSC Administrator address
