·    Go to Forms and rules

·    Select Player Forms

·    Select Out of State Play

·    Print and Complete the form

·    mail to:     Kentucky Youth Soccer Association

                    Attn: Out of State Permission
               443 S. Ashland Ave., Suite 201

                    Lexington, KY 40502

·     Along with:    Copy of Player’ Birth Certificate or Passport

                         Check to 17.50

                         Self addressed stamped envelope 

·     Information needed to complete the form:

                          League: Cincinnati United Soccer League

For Indiana Player

·      go to www.indianayouthsoccer.org website

·      Click on Download Center (from the side menu)

·      You will find the form under Registration Forms

		Resident Player Registration - Permission to play on an Out Of State Team

·      Print and complete the form
