You will need:

		Under Forms and Resources 
		click All OSYSA Forms

		Under tournament Forms

		click on the logo to the right 

			OhioSouth e-Travel Online Form System

		Fill in email address and your street number

		The form needed is the Guest Player Roster Form

		Fill in all information needed.  

For A Tournament Guest Player Who Doesn't Play Select Soccer: 

You will need:

	Membership form from osysa.com 
		Under Forms and Resources 
		click All OSYSA Forms 
		under Other Forms 
		click US Youth Soccer/Ohio South Membership Form for PLAYERS 
	Player's Birth Certificate 
	Picture of Player
