New Team Registration

For new teams only you will need to contact the Club Administrator with:

·    Team Name

·    Date of birth of oldest player

·    Gender of team

·    Coaches name and E-Mail

·    Assistant Coached name and E-Mail

When you get your password from the League, you will be able to start adding your players to the CUSL

Click – Navigation

Click – Organization

Click – Other

Click - team

select our club Cincinnati West Soccer Club (submit)

select your team then enter your password

Select “Add a new Player” to add all your players.

Once your players and Coaches are all added and Information is correct, E-mail the Club Administrator and

request that your Team Cards be printed.  The Administrator will need the Team Name and Number as well

as coaches names and Card ID # to complete this process.

You will be notified when the cards are ready for pick up.

For each player you will need:

·    signed player pass with small picture (glued) (head only)

·    signed player membership form (with DOB Highlighted) from CUSL website "Print Player registration Form"

·    proof of birth date (copy of birth certificate (highlight DOB) or last yr's player card)

·    Player Code of Conduct

·    Parent Code of Conduct

***If you have players from Indiana or Kentucky you must have the required forms from their state

 associations granting them permission to play in Ohio South. (Out of State Players Instructions)  This process could

take 2 or more weeks so plan accordingly.

For each coach you will need:

·    signed coach pass with picture affixed

·    signed coach membership form from  CUSL website "Print Coach registration Form"

·    risk mgt form from www.osysa.com

        Risk Management/Kidsafe (Employment/Volunteer) Disclosure Form

     (If you did this new last yr, you can use a copy of last yr's form)

·    Coaches Code of Conduct

For All Packets you will need

A copy of your roster (CUSL website under reports - print roster)

A copy of the Registration Checklist (found on the website under Coaching Resources)

Follow their instructions for organization of paperwork.

Fill out Registration Checklist.

Add Codes of Conduct.

Return to the Club Administrators house.
