- You should make a copy of the membership form to keep on hand for games and practices
· proof of birth date (copy of birth certificate (highlight DOB) or last yr's player card)
- One form can be signed by both parents
***If you have players from Indiana or Kentucky you must have the required forms from their state
take 2 or more weeks so plan accordingly.
For each coach you will need:
· signed coach pass with picture affixed
· signed coach membership form from CUSL website "Print Coach registration Form"
- U15 and above must list players and the High School team they played on. Coach must sign.
· risk mgt form from www.osysa.com
- Print the page that appears after you click submit
- Make a copy for next years registration (These are good for two years)
For All Packets you will need
A COPY OF YOUR ROSTER (CUSL website under reports - print roster) I will take these out for CUSL rules, but they
are needed for the reviewing process.
and Initialed by the coach or team admin. They will be signed by a 2nd reviewer at our club meeting.
Paperwork should be organized in the order of your roster and Paper clipped in the

All Codes of Conduct for players, coaches and parents should be added to the packet in a separate stack.
These are not necessary for CUSL but are mandatory for CWSC, I will not put your processed cards in the bin until
I receive your codes of conduct.
I DO NOT GET THE CWSC MEDICAL RELEASE FORMS. Those are for you to keep.
Return to the Club Administrators house or bring to the Club meeting.
